II. RELATIONSHIP OF CONSUMER TO TAEC

(To help determine if we have any records about the consumer, TAEC requires information about the type of interactions the consumer has had with TAEC.)

7. Have you submitted a warranty claim to Toshiba America Electronic Components Inc. in the previous 12 months?
8.Have you called, emailed, or made any type of inquiry with Toshiba America Electronic Components Inc. in the previous 12 months?
III. TYPE OF CONSUMER REQUEST

The request is subject to verification of the consumer and/or the agent's identity, and TAEC may contact you for further information in order to verify the request.

TAEC will attempt to verify the requests by matching the consumer's information provided with the information it has on record about the consumer, if any.

TAEC will make all efforts to verify the requests quickly and respond to the consumer's requests within 45 days. In the event verification or response takes longer than 45 days, TAEC will request additional time to complete the request, but no more than 45 more days.
